Entry Level Training Application for Admission
CCJA Form 1
Any agency that desires to enroll a student in an entry level training course including law enforcement,
jailor/court services, dispatcher, or animal control must complete an Application for Admission form.
NOTE: Applicants for the Entry Level Dispatcher or Entry Level Animal control schools should only complete
items 1 - 16 and 26.
This form must be received by the academy no later than two (2) weeks before the
start of the course.
Entry Level Training Medical Examination

Any student who is being enrolled in the entry level law enforcement or the entry level jailor/court
services classes must undergo a complete physical examination within one (1) year of the starting date of the
course in which they are being enrolled. Upon completion of the physical examination, the
examining physician must complete the Report of Licensed Examining Physician.
NOTE: Physical examinations are not required to enroll in the entry level dispatcher or animal control officer
courses.
This form must be received by the academy no later than two (2) weeks before the start of the course.
Application for Academy Membership
CCJA Form 5
The administrator of any agency that desires to become a member of
Cardinal Criminal Justice Academy must complete this form and return it
to the academy director. The director will present the completed
application at the next Executive Board Meeting.
In-Service Credit for College Courses

These forms must be completed by any officer/deputy who desires to
obtain in-service credit for attending college courses. The
forms must be completed, signed and submitted to the Department of
Criminal Justice Services.
RADAR Instructor School

This form must be completed and returned to the Doug Cooley to enroll in the above course
that will be conducted at the Holiday Inn in Wytheville, Virginia.
Driver Training Instructor School
Driver Training Instructor Application
This form must be completed and returned to the Doug Cooley to enroll in the above course
that will be conducted in Danville, VA.
DCJS Request for Training Extension
DCJS Form TE (rev. 10/00)
Any agency administrator that desires to request a training extension for an officer must submit this form. Extensions
may be granted for up to 90 days. Requests must be received prior to the expiration date of the officer's
